Please be advised that each student has the opportunity to enroll in the student
accident insurance program. Student accident insurance protection is being underwritten
by the ACE American Insurance Company, 436 Walnut Street, Philadelphia, PA 19106.

If internet is not available to you, please contact the building secretary to obtain the application and related plan descriptions. The information can be found on the SCASD Home Page (www.scasd.us) Titled “2011-12 Student Accident Insurance”.
Please review the application to determine which coverage you desire, if any. Upon reviewing the coverage, please fill out the application form in the appropriate spaces for the coverage desired. All checks or money orders are to be made payable to: American Management Advisors and mailed to:

Please do not send cash and do not send the application back to your student(s) homeroom teacher. All applications for enrollment to the insurance program should be mailed by September 30, 2011. If you do not want to participate in this program, please disregard this notice.
